Winter 2007 - there are no non-park district classes which may use this form. Please register directly with the park district.

If the student being registered has a sibling in the same class (i.e. Kamp I) then use the regular kit fee for the first sibling and the sibling kit fee for each additional sibling in the same class.

    We will contact you in the event that the class and time you selected are full.
Registration will not be processed without minimum deposit of $50. Balance of class fee and kit fee will be due at first class. Registration is processed on a first-come first-serve basis and will not be processed until receipt of deposit. Payment plans are available upon request. All monies paid to Kindermusik By Colleen are non-refundable. If the class minimum is not met the class may be cancelled and you will be notified by mail and refunded your money. Make-up classes are available during the current session only.

Children enrolled in "Village", "Our Time" and "Adventures - Kamp I" must be accompanied by a parent/caregiver for the entire length of the class. Children enrolled in "Imagine That" and Adventure Kamp II must be accompanied by parent/caregiver during the final 15 minutes of class. Children enrolled in "Young Child" must be accompanied by parent/caregiver during the final 10 minutes of class. Children enrolled in Adventure - Kamp III are unaccompanied by an adult.
